Walk into Vertex's Boston HQ in 2028 and the org chart tells you everything. Cystic fibrosis, the disease that built this company and paid for everything else, is one of five commercial franchises instead of the whole business. TRIKAFTA and its successor ALYFTREK still print roughly $11-12B a year with 90%+ margins because there's no competition and won't be. But sitting next to it: JOURNAVX doing $2-3B in acute and neuropathic pain as the first non-opioid alternative that actually works, CASGEVY finally hitting scale as authorized treatment centers clear the throughput bottleneck (roughly $1.5B run-rate by late 2027), povetacicept generating $1B+ in IgA nephropathy with the label expanding into lupus nephritis, and inaxaplin approaching approval in APOL1-mediated kidney disease. Revenue trajectory looks like $13B this year growing to $18-20B by 2028, and the mix shift matters more than the number.
